What types of Dental Plans can I purchase through USI Colburn?

USI Colburn offers an excellent dental plan to our Association members through United Concordia. The program is called Concordia Flex and it is a fee-for-service dental program with a network of contracted dentists called Participating Dentists. Members have the freedom to use any licensed dentist. We offer two benefit levels, Basic and Comprehensive. Payment will be based upon United Concordia’s schedule of maximum allowances. If the member utilizes a Participating Dentist there will be no claim forms to fill out. For more information contact a Representative at 1-800-265-2876.
