If my coverage is terminated am I eligible for conversion?

Depending on what type of coverage you have you may be eligible for conversion. If you have Keystone you may apply for continuation of Keystone coverage if your application is made to Keystone within 30 days of when you became ineligible for group coverage. If you have Personal Choice you may be eligible for conversion if you or your dependent have been continuously covered under the group contract for at least three months, or covered for similar benefits under any group plan that this group plan replaced.
