How do I add my newborn to my policy?

If you wish to add your newborn to your policy simply fill out the application with the name, date of birth and Social Security number of the baby. The application must be received at the carrier no later than 31 days after the birth. Please send this form directly to USI Colburn Insurance Service, 1 International Plaza, Suite 400, Philadelphia, Pa. 19113, so that we can update your policy. We will process the change to your policy.
